This is just my beryl modification of the compiz theme included in the Neutronium theme pack from nanoflower. All the credits go to nanoflower! See http://gnome-look.org/content/show.php?content=46153
I've only adjusted the shadows and buttons vertical position. Looks good with Neutronium GTK2 theme or dark coloured QtCurve as seen on the screenshot.
To install import it via the Emerald Manager. By default it uses only two buttons (minimize and maximize) byt you can of course customize it. Last changelog:

I dont understand how you use the GTK theme with beryl, whenever I go to Preferences -> Themes, and select my GTK theme, nothing happens. (To avoid confusion, how did you get your very top menu bar to look like it did.) Thanks
Are you on Edgy? If yes, put the gnome-settings-daemon to the sessions>autostarted programs.
some themes causes this problem. I have a same problem if VaterVapor or GlossyP theme installed. Remove the installed themes step by step and try to change your GTK2 theme.

use the emerald-theme-manager to edit the theme and set a pixmap to the close button ;)
hi, that's simple. in the Emerald Theme Manager go to the "Edit Themes" tab -> "Titlebar" tab and in the Title-bar Object Layout input filed just add "C" next to the "R" in Default Layout or choose Normal Layout from the dropdown :)
